If I earn just above the 20% tax limit of £42,476, will I pay 40% only on the amount above £42,476 or all the amount?

Just on the bit above it.

Don't forget that your taxable income is decreased by the amount you pay into a pension scheme, an approved share investment plan or e.g. the bike to work scheme.
